What are the novel predictors for wound healing? What is the ideal model of care for people with chronic wounds?

Phase 1: retrospective audit of the routine collected data (including malnutrition score, frailty score) and assess its association with outcomes of interest (healing time, wound-related hospitalisation)

Phase 2: prospective measuring of PROMs, cognitive profile, survey to identify unmet needs, with the aim to inform the ideal design of the model of care for people living with chronic wounds.

Phase 3: Implement improved model of care and evaluate
